INSTRUCTIONS FOR SUBMISSION OF ON-LINE APPLICATION
|Read the Instructions carefully and select "I Agree" and Press 'REGISTER' button to proceed further.
|For detailed Notification, click here and read it carefully before filling-up the on-line application.
|Before start of filling-up of application through on-line mode, the worker should keep ready, the following details/ documents:
- Registration Number & Date of Birth.
- Valid E-mail ID & Mobile Number.
- Scanned copy of the recent passport size color Photograph. Worker should ensure that the photograph is clear and visible. Photograph should be in Jepeg/jpg format. The size of document should be greater than 20kb and less than 50kb.
- Aadhaar Card.
- Bank Passbook.
- Latest renewal slip.
- Labour Card/Regsitration Cerificate (It is not required for the woker who is already avaible in the online system).
|Scan Documents should be in pdf/Jpeg/jpg format. The size of document should be greater than 100kb and less than 200kb. Aadhaar Card once filled by worker in the on-line application form will not be changed.
|Worker can apply the scheme only after updating the his profile.
|How to Apply:
|Worker should have a valid personal e-mail ID and mobile number. It should be kept active any notification may recieve on this registered mobile number and email.(please ensure that email sent to this mailbox is not redirected to your junk / spam folder).
|Woker should take utmost care to furnish the correct details while filling in the on-line application. YOU CAN EDIT THE INFORMATION BEFORE SUBMISSION OF STEP-I and STEP-II. Once the form is submitted, it cannot be edited.
|The step by step process for Applying Covid assistance scheme is given below:
Please visit the website http://manipurbcwb.in/ then click Apply of Covid Assistance Scheme. Then check your registration number. If it is available at online then Login to apply using registration number and date of birth.
Step-A: Fill all the Personal Details,Contact details, bank details and upload the supporting documents. Don't forget to save the fill record before pressing next button. Atfer completion of filling data press the Final Submit button
Step-B:You can log out and re-log in to continue to the Application Form for the incomplete details and documents.
Step-C: Click on the claim button to apply for Covid Assistance Shceme.
|once apply cannot apply again and the selection process in future.
|THOSE CARD HOLDER'S WHO RECORD ARE NOT AVAILABLE ON ONLINE
|The worker agreeing to the terms & conditions may proceed to claim the scheme.
|The worker should fill up all the desired information i.e. Personal Details, Contact Details, Bank Details etc. correctly.
|Upload the passport photograph and relevant documents. Please note down the reference number for future use.
|The candidate has to log-out and log in again in order to fill up other details in application form. Press final submit button to complete the apllication.
|RIGISTERD WORKER AVAILBLE IN ONLINE
|After registration, candidate has to login and complete other details in application form like Personal Details, Qualification Details, and Declaration etc.
|Instructions regarding scanning of Photograph and Documents: Candidates should upload the scanned (digital) image of their photograph as per the process given below. The applicant should note that only jpg/jpeg format is acceptable:
i. Photograph Image:
- Photograph must be a recent passport size colour picture.
- The size of the scanned image should be between 30kb -50kb in jpg/ jpeg format only
ii. Scan Documents :
- Please scan the relevant Certificates.
- Each documents in JPG/JPEG/PDF format only and Size of file should be between 100kb-200kb
|If the wroker completed filling the his profile then click on final submit button to complete updating profile.
|Once the application is submitted, candidates will automatically be redirected profile page and worker should apply for Covid Assistane scheme by clicking on Claim button.Candidate may generate acknowledge slip for future reference.
